    I-CAN is a program that provides pre and post release services to individuals incarcerated in the Department of Correction. I-CAN serves thousands of people in DOC custody, with an emphasis on reducing idleness, violence, and recidivism, while promoting a successful transition to the community.

    Position Summary:

    The Fortune Society is seeking an experienced Health Home Care Manager to build and manage the performance of our Health Home activities in the Bronx, and surrounding areas. S/he must be HARP certified and fully versed in the NY State policies and procedures as they relate to Health Home care management agencies. The Care Manager will have overall day-to-day responsibility for coordinating the activities of the care team for the patients with complex medical and/or psychiatric conditions, and for facilitating the patients’ access to the full range of medical and mental health services they require quickly and efficiently.

    The Fortune Society is seeking an experienced Health Home Care Navigator to ensure the successful implementation of the care coordination efforts of our Health Home activities in the Bronx. S/he will utilize health information technology to monitor and track pertinent client health information and will support the Care Manager by participating in client intake assessments and client outreach and engagement. Field visits are required.

    Position Summary:

    The Intake Coordinator/Resource Specialist assesses each client’s eligibility for Fortune services by thoroughly screening each client, completing the agency’s comprehensive intake package, discussing Fortune services and the eligibility criteria for each, and making appropriate referrals to all Fortune programs and services. In addition The Coordinator will need to gather resources in order to assist our clients with appropriate referrals.
