    Position Summary:

    The Fortune Society is seeing highly-motivated health services candidates to join our newly formed Recovery Coordination Agency practice (RCA). Our mission-driven RCA Assessment Specialists will serve as the front line experts on screening, assessing and linking both criminal justice involved and general population individuals with chronic behavioral health and medical conditions to Home and Community Based Services (HCBS).

    The screening and engagement of potential clients will mainly take place in the community, and will include working with shelter staff, parole, hospital and other in-patient discharge planning teams, and various community-based partners, all with the goal of HCBS connectivity and Health Home enrollment for all Health and Recovery Plan (HARP) members.

    This is an excellent opportunity for new entrants to the health services field who are looking for exposure to a diversified client base and who will thrive in a client-centric, entrepreneurial, performance-based environment. Under the supervision of the Recovery Coordination Supervisor, our assessors will be trained in best practices for making meaningful health and HCBS connections. This is absolutely a growth position within the health services field for the right candidate(s).

    The Transitional Specialist assists HIV+ participants in the transition from Riker’s Island or any NYC DOC facility to services at The Fortune Society or other social services agency.
