    The Family Services Case Manager aims to increase awareness on the importance of fathers in the lives of their children; while strengthening family bonds. The counselor is responsible for engaging non-custodial fathers in our Fatherhood program, though case management and parenting groups.

    Working under the supervision of the AVP, the Program Supervisor will be responsible for the overall day-to-day functions of the scattered site housing program to ensure continuity of services for housing clients making the transition from incarceration and homelessness to independent living. 

  • Care Navigator

    Position Summary:

    The Care Navigator ensures successful implementation of the care coordination efforts of the Care Management Unit by communicating and collaborating with the Care Manager/Care Management Team. The Care Navigator utilizes health information technology to monitor and track client health information and supports the Care Manager by participating in client intake assessments and client outreach and engagement. Field visits are required.

    Position Summary:

    Responsible for working with court stakeholders to assess and screen, and advocate for individuals to be placed in Fortune’s Alternative to Detention (ATD) or Alternative to Incarceration (ATI) programs, or Abusive Partner Intervention Program (APIP); and report to the court regarding clients’ participation in programming.
