  • Outreach Worker

    Position Summary:

    Under the supervision of the Outreach and Engagement Manager, the Outreach Worker will be responsible for conducting outreach and recruitment efforts to identify eligible program participants and connect them with appropriate staff for services, managing relationships with program partners that are part of the program’s referral network, and collaborating with other Fortune departments offering supportive services. The Outreach Worker will also be responsible for working closely with Admissions team for the purpose of scheduling intake interviews.

    Peer Community Health Worker is responsible for escorting participants to various appointments and assisting in picking up newly released individuals from Rikers Island and other DOC facilities as needed.
