Fortune Society Gets $400K MRT Supportive Housing Grant

Formerly incarcerated individuals who are homeless and suffer from chronic medical conditions will now have increased access to Fortune Society’s scatter-site housing and essential case management services, thanks to a new grant from the NYS Department of Health (DOH) Medicaid Redesign Team (MRT) Health Homes (HH) Supportive Housing Pilot Project.

The $400,000 grant will enable Fortune Society to provide 20 additional units of scatter-site housing and comprehensive case management services to individuals with criminal histories who are enrolled in NYS’ Health Home Program. Specifically, the funding will support individuals who are high-cost Medicaid consumers with two or more chronic health conditions, for example, mental health, substance abuse disorders, asthma, diabetes, heath disease or obesity. Individuals who live with HIV/AIDS or a serious persistent mental illness are also eligible.

This new funding will also allow Fortune’s Housing Specialists to work with 20 more homeless high need men and women to create a housing service plan that focuses on needs of eligible individuals and presents goals to maintain housing stability and achieve the best outcomes.

“Without support and housing, many individuals recently released from incarceration are disconnected from the care they so desperately need,” said JoAnne Page, President and CEO of The Fortune Society. “This is particularly true for those who suffer from chronic illness. Incarcerated individuals have rates of serious mental illness that are two to six times higher than the general population, and have rates of substance use disorders that are more than four times that of the general population. It’s crucial that we intervene immediately — offering a place for them to live and receive help. We are grateful to New York State Department of Health for helping us to provide the best possible care to our clients who need it the most.”
